BeautiBank Membership Agreement

BeautiBank is an exciting membership program offered by Portrait Health Inc. (“Portrait”) that gives clients the opportunity to accumulate funds to use towards services and obtain exclusive offers redeemable through Portrait-affiliated aesthetics businesses.

Terms and Conditions

  1. Monthly BeautiBank Membership Fee payments shall be made in advance by direct debit from the member’s designated credit/debit card, which shall be kept on file by Portrait.
  2. The Monthly BeautiBank Membership Fee is $99.00.
  3. Portrait reserves the right to make adjustments to the BeautiBank Membership Program. Members will be given at least thirty (30) days notice of any changes regarding: 1) increase in monthly BeautiBank Membership Fees; 2) a change in the date of the automatic withdrawal; or 3) change in the BeautiBank Membership term.
  4. Gift cards may not be redeemed to pay for any portion of the monthly BeautiBank Membership Fee.
  5. The initial membership term is twelve (12) months (“Initial Term”). The BeautiBank Membership Fee will be deducted upon the BeautiBank Membership Start Date and will be automatically withdrawn monthly thereafter. Membership will automatically renew for an additional twelve (12) months at the end of the Initial Term (“Renewal Term”) or any subsequent Renewal Terms, unless terminated in accordance with the BeautiBank Terms and Conditions set forth herein.
  6. You have the right to terminate your BeautiBank Membership by providing written notice to hello@portraitcare.com at least thirty (30) days prior to the end of the Initial Term or any Renewal Term (“Termination Period”). Membership can only be canceled at the conclusion of the Initial Term or the conclusion of a Renewal Terms. Any accumulated funds within your BeautiBank will remain available after termination of your BeautiBank Membership.
  7. Funds can be used for treatments performed by any Portrait-affiliated provider at any Portrait-affiliated location. Funds cannot be used towards retail sales, for example, at-home skincare products.
  8. This Agreement is specific to the member and may not be assigned or transferred and any accumulated funds can be used only by the undersigned Member.
  9. The Initial Term will commence on the date Member makes the initial BeautiBank Membership Fee payment.
  10. Member is responsible for ensuring that the credit/debit card number on file is valid so that the monthly BeautiBank Membership Fee can be charged to the card. You may change the card information provided at any time by contacting hello@portraitcare.com. It is your responsibility to ensure there is always a valid credit/debit card on file at the time the monthly BeautiBank Membership Fee is charged. If a valid credit/debit card is not made available, Portrait may exercise all rights available, whether under this Agreement or applicable law, which may include suspension of your BeautiBank Membership until payment is made, assessing a late payment fee of $25.00, and/or termination of this Agreement.
  11. Portrait reserves the right to terminate or suspend BeautiBank Membership, at its sole discretion, due to breach of the terms of service, if payments due by the Member are not paid for thirty (30) days after the payment due date, or the member knowingly provides false or misleading information to Portrait or any of its providers or staff. If Portrait terminates this Agreement for any reason, Portrait reserves the right to retain any accumulated BeautiBank funds to cover any reasonable costs it may have incurred as a result.

By signing below, I authorize Portrait (identified below) to charge the monthly BeautiBank Membership Fees to my financial institution via Electronic Funds Transfer service with the credit/debit card information I have provided. I further acknowledge and accept all BeautiBank Terms and Conditions.

I understand that I am fully responsible for my payment, and if at any time I decide to make any changes to my credit/debit card information, I must contact hello@portraitcare.com to provide updated card information. Change of payment method will not affect other provisions of this Agreement. If I decide to terminate my membership, I understand I am subject to the termination Terms and Conditions set forth herein and must provide written notice thirty (30) days prior to the end of the Initial Term or any Renewal Term. I understand that no refunds will be issued after the BeautiBank Membership Fee has been charged. I further understand that BeautiBank Membership is non-transferable and BeautiBank funds may not be shared. Portrait reserves the right to change BeautiBank policies and pricing at any time upon providing reasonable notice.
